RN Involvement — A registered nurse leads the development of the care plan for all of our clients with dementia. Our experienced nurses create an individualized plan of care and provide oversight of the direct care staff to ensure the safety and comfort of your loved one. Dementia affects everyone differently and this difference is acknowledged and addressed by our care coordinators as they work with client and client’s family.
Trained Staff — All staff who care for clients with dementia have been certified in the Brighthope Homecare person-centered approach to memory care. This allows the staff to assist clients with their daily routine, provide mental stimulation, and encourage participation in activities of daily living. Our staff will also provide regular summaries of the clients routine and behavior and promptly discuss any concerning behavioral changes to the family. They understand to communicate changes to the nurse so the care plan can be revised based on the clients’ current state. Collaboration is essential in the care of your loved one.
